Congenital Heart Disease: The Transition From Pediatrics to Adult Care

April 28, 20231. If I was born with a heart defect, why is it important to seek care into adulthood?
Our health needs change as we age. New health needs come up with aging especially having had heart procedures. Continued monitoring is needed to prevent any unneeded hospitalization or urgent procedures. The transition program at VCU Health System prepares young adults and their families for this continued care, even if they are young and healthy. These programs are important to prevent gaps in care
2. How does the transition program help me to advocate for myself and my care?
Transition programs empower YOU through education and support related to you and your individual health needs. We work to understand your goals, interests, and concerns to help you make informed decisions with your healthcare provider.
3. What kinds of things will I learn with transition education?
The transition program at VCU Health prepares you for all of life's major events. This includes pregnancy, exercise, and career choices. We also guide you on insurance needs, social support, and medication coverage. Education is focused on giving you the tools to empower your decisions and keeping you healthy.
4. Will the adult team continue to work with my pediatric team during and after transition?
Here at VCU Health, our pediatric cardiology team and adult congenital cardiology team work together throughout the transition process. Typically, at age 16 the family is introduced to the adult team while at a visit with pediatric cardiology. From there the transition program starts. This includes multiple appointments from the ages of 16-18 with the patient and family members. At the age of 18-21, once everyone is ready, transfer to the adult team occurs. The connection with pediatric cardiology is still very present throughout this process. The adult and pediatric team work together to help guide our patients.
5. Why will I need frequent appointments when I am receiving transition education from the adult team?
There is a lot of information to cover during transition. It gives the adult team the opportunity to get to know you. We use the time to discuss your concerns, personal goals, and your heart to care for YOU as a whole. It takes families time to build trust in a new group. This time investment has been proven to be beneficial for the patient and families.
